We are looking for a Carer Wellbeing Facilitator to join our team!

Hours: 35 per week
Salary: £22,227 per annum
Location: Hybrid (Office and Home) - We are currently testing hybrid working with Tuesdays, Wednesdays and Thursdays office-based and Mondays and Fridays home-based.
Reporting to: Team Leader Core Services
Probation Period: 3 months
Leave Entitlement: 28 days plus statutory Bank Holidays.

Specific objectives of the post include;

  • Empower carers to be stronger and more confident, especially in controlling their life, claiming their rights, realising their potential, achieving their goals, and staying healthy mentally, physically and financially.
  • Work with carers on a one to one basis, using an asset / strengths-based approach, to ensure carers are best placed to understand the options available to them to make good decisions on presenting challenges and to be better placed to meet those challenges themselves in the future.
  • Efficient and effective management of personal caseload.
  • To contribute to the development and delivery of group-based work (e.g. facilitated peer support groups) both at GCA and at community venues.
  • To provide support to carers across the wellbeing themes of emotional resilience, personal development, connectivity and health using a strengths and assets-based approach.
  • To complete statutory Carers Assessments and support plans.
  • To develop and maintain links with key stakeholders including voluntary organisations, general practitioners, Primary Care networks and Gateshead Council.
  • Any other duties that the Chief Executive Officer requires which are commensurate with the post.

Person Specification


  • At least 12 months’ experience (paid or voluntary) of working within the Health and Social Care sector.
  • Experience working with people facing multiple life challenges.
  • Experience in managing a high and varied workload.
  • Experience in assessing peoples’ needs.
  • Home environment which has a suitable and private space for home working.


  • Knowledge of the Care Act 2015.

  • Experience in working with people using a ‘strengths and assets’ based approach.

  • Lived experience of providing unpaid care.
  • Experience in using Client Record Management systems.
